SEVERT SUPER SMILE STARS GUIDELINES
We hope this program will be an enjoyable way to help motivate and reward you during your orthodontic treatment. Good luck earning your SEVERT STARS!
SEVERT STAR RULES
- SEVERT STARS are only awarded at regularly scheduled appointments. Walk-in or emergency visits do not apply.
- The number of SEVERT STARS you earn at each appointment is up to you. There is no limit!
- The SEVERT STAR prize cabinet is located near our front desk. The prizes may change from time to time. Not all prizes will always be available (prizes change periodically).
- Once you spend your SEVERT STARS on a prize, they are spent. You must earn more SEVERT STARS to purchase more prizes.
- If your SEVERT STAR card is lost, the STARS are lost as well. They are like money and cannot be replaced.
STAR GUIDELINES
1 SEVERT STAR
- Keep your regularly scheduled appointment
- Arrive on time for your appointment
- Good Oral Hygiene - your teeth are clean and your gums are healthy
- Nothing loose or broken (this means brackets, bands, removable appliances, etc).
- Wear your Dr. Severt T-shirt to your appointment
5 SEVERT STARS
- Good grades. You must make all A's and B's. Bring your report card to your appointment
- Raise your grades 1 full letter grade (i.e. C to a B). If any grades go down a letter grade, you will not receive any SEVERT STARS for "improved grades" when they go back up.
- Proof of community service or charity work -- anything you do to better the community around you. Please bring official verification of your service work from the sponsor or organization.
- Wear your Dr. Severt T-shirt out and about (at the mall, movies, baseball games, etc.) and be seen by Dr. Severt or any of our staff
10 SEVERT STARS
- Visit your general or pediatric dentist for your regular check up and cleaning. (Bring a signed note or receipt from your dentist.)
20 SEVERT STARS
- Our thank you for referring your friends to our office. Be sure they tell us you referred them. TWENTY SEVERT STARS to be awarded to you after their first visit.
